Step 1:
	COMPLETE the IRA Account Opening forms. Download these documents
	Sign & Return to:

	READ & RETAIN Make sure to review the Account Opening and IRA Adoption agreements and disclosures (.pdf) Download these documents
Step 2:
	REVIEW the SecurePath by Transamerica Client Brokerage Account Fee Schedule. Download this document

Step 3: Call Transamerica Retirement Services to request your Direct Rollover Form.

Step 4: Once you receive your Direct Rollover Form, mail one signed copy to Transamerica Retirement Management. Or, you may fax the forms to 800-503-8884 to expedite the account opening process. (If you choose to fax the forms, the original signed documents must still be mailed to the address above for our records.)

Step 5: Your Individual Retirement Account will be established once we receive your Account Opening and Direct Rollover forms. A SecurePathSM by Transamerica financial advisor will then call you to discuss your investment options.
