Documents Needed When You Retire
- Social Security Number (SSN)
- Birth Certificate (original or certified copies by issuing agency)
- W-2 or self-employment tax return for previous year
- Military Discharge papers, if applicable
- Spouse's birth certificate and SSN if he/she is applying for benefits
- Children's birth certificates and SSN's if applying for them.
- Proof of U.S. citizenship or lawful alien status if you (or spouse or children) were not born in the U.S.
- Name of bank or other financial institution and account number for Direct Deposit
